Application form

The Application form is in Microsoft Word format. Clicking on the link below should open the file in your browser window. You can then enter your details on the screen and print it out. Unfortunately we cannot accept the forms on line because we must have a signature showing your acceptance of our Membership Rules and Code of Conduct (included on page two of the form).

We also require a passport style photo or digital image for the Membership/ID Card. Digital photographs can be emailed to admin@southeast4x4response.org.uk.

The second file is a templete letter. This can be printed out and sent to your insurance company to obtain written confirmation of your cover for voluntary activities.

The completed form, passport photograph (if not sending a digital image) and membership fee should be sent to the address at the top of the form. Please make sure you sign the second page and send both pages to us!
